Medical RS's are not awarded until they're petitioned for. You're not eligible to apply for a Med. RS until you've exhausted your eligibility (with one exception, discussed below). Med. RS are not awarded to everyone who applies. If you were RS due to an injury, you'll most likely be given a 6th year of eligibility. If you're RS'd for size/grades/development, then it's actually pretty hard to get the medical hardship. The rule says that you can get a Med. RS for a season if you've played less than 10% of the games. In football, that's about 1 game.
Now, there is an exception for the Med. RS. Jamal Golden was actually awarded a Med. RS for 2013. The reason he applied for a Med. RS and was awarded it before his eligibility expired is because he exceeded the 10% rule. If I remember correctly, he was injured in the 3rd game that season. He applied for and was granted a Med. RS for than season.
