Notice this is 4th Q and they have 4 people in the back field. That is what burning opponents with the pass accomplishes.... and sets up the run game.

The beauty of the play is that since it was a forward pass, it couldn't be a fumble, only an in-completion.

If it was a forward pass though, should there have been a flag for ineligible downfield linemen?

No they are given 3-4 yards past los.

Must be forward pass the goes past the line of scrimmage. You can have players 15/20/50 yards down field on screen pass.

This too. But if it does go past los they are generally given 3-4 yards (officials are relatively easy on this unless its really obvious)
